Strategic Vacancy for Community Health Professionals
The Health Collaboration Center (CCS), a reference organization in the implementation of large-scale public health projects in Mozambique, is recruiting dedicated professionals for a high-impact position. An Opportunity for Real Impact
As a Case Manager, you will be the essential link between the community and the Health Units. Your role is vital to the success of the project, working on the front line of the response to HIV and TB. You don't just manage – you facilitate, direct and ensure continuity of care, transforming protocols into saved lives.
Day to day life is dynamic and involves direct field work, mentoring for community actors and close collaboration with those responsible for the Health Unit and SMI-CCR. We believe that excellence in case management is the cornerstone to reducing treatment abandonment and improving outcomes for patients.
Primary Responsibilities
Selected professionals will assume a comprehensive set of responsibilities that challenge and develop clinical and management skills:
- Implementation and Mentoring: Conduct community liaison activities, ensuring that public health protocols are executed effectively and compassion.
- Logistics Coordination: Work closely with the different actors of the Health Unit, including the SMI-CCR, to optimize care flows.
- Referral Management: Facilitate the reception and efficient referral of referred patients from the community, ensuring that no one is left behind.
- Patient Support: Provide continuous support and follow-up to patients throughout their journey of treatment.
Requirements and Ideal Profile
We are looking for candidates who combine technical training with exceptional interpersonal skills. Our ideal profile includes:
- Academic Training: Minimum of 12th grade completed.
- Specialized Knowledge: Solid command of HIV and TB topics, covering forms of transmission, prevention and treatment.
- Communication Skills: Excellent verbal and written communication in Portuguese, essential for interacting with patients, families and teams techniques.
- Local Knowledge: Knowing how to speak the local language(s) of the district or province is a significant advantage, facilitating community outreach.
- Systemic Vision: Detailed knowledge of the flow of care for HIV, TB, SMI and other related procedures in the Health Unit.

Hiring Details and Work Location
The vacancy is for two positions, on a full-time basis, with a contract lasting until December 30, 2026.
The work locations are in the following districts of the Province of Nampula:
- Memba District: At the Mazua Health Center.
- Nacala Porto District: At the Mathapué Health Center.
