About the Role and Impact on Our Project

As a Case Manager, you will be the key player in implementing and mentoring community connection activities. This isn't just a job; It's a mission. You will work on the front line, ensuring that patient referral and counter-referral flows work efficiently and humanely.

Your day-to-day life will be dynamic and deeply connected to the community reality. You will work closely with those responsible for the Health Unit and SMI-CCR, facilitating the reception and referral of referred patients from communities, ensuring that no one is left behind in accessing essential HIV and Tuberculosis care.

Key Responsibilities and Expectations

  • Facilitation and Liaison: Act as an effective liaison between services health and communities, promoting the continuous flow of patients.
  • Mentoring and Support: Guide and support patients throughout their treatment journey, offering psychosocial and practical support.
  • Specialized Knowledge: Apply your knowledge on the topics of HIV and TB on a daily basis, including forms of transmission, prevention and treatment.
  • Teamwork: Integrate with collaboratively with the different actors of the Health Unit, ensuring a unified and patient-centered approach.
  • Registration and Reference: Ensure that all referral processes are correctly documented and routed, contributing to the accuracy of health data.

Ideal Candidate Profile

We are looking for someone with an unwavering commitment to public health and a solid knowledge base. Your ability to communicate and empathize with the community will be as important as your technical skills.

Mandatory Requirements

  • Academic Training: Minimum of 12th grade completed.
  • Technical Mastery: Excellent understanding of HIV and TB themes, with a focus on transmission, prevention and treatment.
  • Communication: Excellent verbal and written communication in Portuguese.
  • Local Knowledge: Ability to understand and navigate the flow of care for HIV, TB, SMI and other related services in the Health Unit.

  • Languages Locations: Knowing how to speak the local language(s) of the district or province where you are located is a great advantage, facilitating community rapprochement and trust.
  • Adaptability: Ability to deal with varied challenges in the field with professionalism and resilience.

Vacancy Details and Conditions

CCS is offering employment contracts with a defined duration until December 30, 2026. This is an opportunity to join a project with international funding and a committed team.

  • Available Vacancies: 2 (Two) positions.
  • Work Locations: The functions will be performed in the districts of Memba (CS de Mazua) and Nacala Porto (CS de Mathapué), in Nampula Province.
  • Work Model: In-person (on-site), in direct contact with health units and communities.
  • Type of Contract: Full-time (full-time), with total focus on the project.
  • Remuneration: Negotiable, according to the candidate's experience and qualifications.
